Pre and Post Surgery Questionnaires

At my practice, I aim to provide care that is tailored to each patient's individual needs. To help me achieve this, I ask all my surgical patients to complete pre and post surgery questionnaires. Before your operation, you will be invited to complete a pre-surgery questionnaire, which asks about your current symptoms, mobility and ability to perform daily activities. After your operation, you will be invited to complete similar questionnaires at key points during your recovery. These help me understand your healing; pain levels; return to function; and overall satisfaction with the procedure. Your responses help me in two important ways throughout your treatment journey with us. First, they allow me to track your individual progress and adjust your care if needed. Second, they help my team continually learn and improve the service we offer to future patients. Completing the questionnaires only takes a few minutes, and your responses make a real difference.
